Please complete and submit this Package Request form, to notify us of your interest. We will send you our Letter of Agreement (via fax or e-mail) specifying the service you requested. ( The Letter of Agreement is an PDF document. You may use the Adobe Accrobat link we have provided below to download a free version of the reader.) The signed and dated Letter of Agreement with a check in the amout specified must be received by us to activate your login ID. You will receive an email (or fax if requested) response when your payment is received, which will include your login information.

Package A: The client may submit up to a maximum of 6 inquires during the period of the Agreement. The price for Package A is $ $240

Package B: The client may submit up to a maximum of 15 inquires during the period of the Agreement. The price for Package B is $ 525

Package C: The client may submit up to a maximum of 25 inquires during the period of the Agreement. The price for Package C is $ 800

Package D: Our "Consultant On Call" package requires an initial Retainer in the amount of $ 500 The per inquiry charge is $ 45 Please read the package description for more details.
