Please follow these simple instructions while ordering practice enhancement and compliance products from The Sage Associates.

  1. Complete and send this order form. 

  2. For quantities exceeding 5, select "Other" in the Quantity Box and then specify the quantity you desire in the Comments box.

  3. Upon receipt of your order we will send you an Invoice for the amount due which will include shipping and packaging charges.

  4. All products will normally be shipped within 10 days of receipt of the paid Invoice.

  5. Product satisfaction is guaranteed! If you are not satisfied, return the product(s) within 15 days and product charges will be refunded!
